“This is the story of Jessica Macleod, a nurse practitioner, and four patients she cares for in their homes. With her laptop, satchel and stethoscope, Jessica makes house calls, typically seeing 8-10 patients per day, many of whom live at the outer edges of our healthcare system, unable to visit a doctor’s office due to a combination of multiple chronic conditions, functional impairment, and poverty. Jessica’s work places her in the center of important healthcare issues facing our country – how to take care of people who fall through the cracks, how to navigate end-of-life care, the confusing maze of health insurance, and the burdens placed on families.” To complete this assignment, you will watch the documentary “The Invisible Patients: Life at the Edges of the American Health Care System.” This award-winning documentary by Patrick O’Connor, follows the lives of 4 persons for whom Jessica provides care. The issues raised in the documentary reflect all the key topics discussed in this course. This paper should be written in APA format (title page; citations; references; use of headings; appropriate font size; double-spacing, margins). No abstract is needed. The analysis should be between 3 and 5 pages, NOT including the title page and references. Use correct grammar, spelling, and sentence/paragraph structure. Your analysis of the documentary should include the following points: 1. Opening: What did you learn from the documentary? Were there any ways in which you identified with Jessica? 2. Content questions: a. There are no norms for clients with chronic illness. What does this mean and how does it apply to each of the stories (Roger, Ron, Wink and Patty) in the documentary? b. Early in the documentary, Jessica asks Roger if he has any spiritual beliefs, and he says that he “respects some of it, but not all of it.” As his disease progresses, his beliefs seem to change. Wink thanks “the good Lord up above” for feeling better, and later tells a story about someone from his church “putting his hand on” his stomach to make the pain go away. How did spirituality and spiritual care influence Roger and Wink’s quality of life? c. At various times, Roger and the Sherrill’s (Wink and Patty) struggle to navigate the “healthcare system.” Medicaid will not pay for the non-invasive vent that would make Roger’s breathing easier. Patty is unable to find new doctor after failing a drug test and then she is dropped by her insurance provider. Discuss the effects of poverty in healthcare. What challenges do the poor face that others do not? What might be done to insure the poor have better access to healthcare? d. During the scene where Roger’s mother tells Jessica that his doctor “didn’t listen to him,” Jess tells us that she feels patient care is often influenced by the provider’s perception of the patient and things like financial status or education. Did you perceive any labelling in any of these persons experiences? If present, how did it influence care management? e. The bond between Roger and his mother, Shirley, is incredibly strong and is clearly a positive factor in Roger’s care. How do you think Roger’s story would be different if his mother was not his caregiver? Ron and his older brother Louis live in the same house, and though Louis views himself as a caregiver (his “mother-brother”), it does not appear to Jessica that Louis provides much care for brother. At the end of the film, we find out that Louis dies shortly after filming completed. Do you think Louis was ever suited to be Ron’s caregiver? f. There is currently a debate about the role of advanced practice nurses in the chronic care model, with many advocating for more independence.
