Complex diseases can be challenging to manage. Effective disease management involves incorporating a patient-centered approach to care. Patients and families are encouraged to be an active part of developing, implementing, and evaluating the plan of care. When detection, diagnosis, and treatment moves from the individual to population management, care can become exponentially more complicated, and often requires a different and more comprehensive approach. In the context of a biopsychosocial approach, health care providers are tasked with implementing collaborative and interdisciplinary strategies to care. The effective and efficient management of population health means collaborating with community entities and includes patients, families, health care organizations, community organizations, and associations. In addition, a biopsychosocial approach to care includes incorporating evidence-based practice and standards as well as the regulatory and national health care initiatives as considerations in care planning. For your initial post, select a population and a complex disease (this could be the health issue and population you wrote about in your Unit 3 assignment, the disease you chose for the first discussion of this unit, or another appropriate disease and population) and address the following.
