The paper will be a minimum of 12 pages in length, excluding title page and references. The paper must be constructed according to APA format 6th Edition. The paper must: Discuss a patient problem (Patient Problem: Are adults with seizure disorders at a higher risk of developing depression compared to the general population without seizure disorder?) Apply Transition Theory to the patient problem in a case study format. (_88​ ​years​ ​old​ ​male​ ​who​ ​initially​ ​presented​ ​to​ ​Good​ ​Sam​ ​hospital​ ​with​ ​altered​ ​mental​ ​status. He​ ​normally​ ​lives​ ​alone​ ​completely​ ​independently.​ ​His​ ​neighbors​ ​heard​ ​several​ ​loud​ ​screams which​ ​prompted​ ​them​ ​to​ ​call​ ​EMS.​ ​Upon​ ​arrival​ ​the​ ​patient​ ​was​ ​found​ ​covered​ ​in​ ​feces​ ​and blood.​ ​En​ ​route​ ​and​ ​in​ ​the​ ​ED​ ​he​ ​has​ ​a​ ​witnessed​ ​Generalized Tonic Clonic seizure​ ​lasting​ ​several​ ​minutes​ ​which​ ​ablated with​ ​2mg​ ​of​ ​ativan. He has past medical history of depression, atrial fibrillation, hypertension, bovine aortic valve replacement, interstitial lung disease and hypothyroidism. ​ Relate one of the Institute of Medicine (IOM)/Quality and Safety Education for Nurses (QSEN) competencies to the patient problem. Students are to utilize the information and knowledge accumulated in the writing of the Clinical Journal, and to develop this knowledge to the next level in the Capstone Paper. The student is encouraged to use the same patient problem and research article used in the Clinical Journal. If the student wishes to select another patient problem for the capstone paper; the student must gain approval of the topic from their clinical faculty member. The paper must relate the patient problem to one of the following IOM/QSEN competencies. The QSEN competencies are listed below: Patient-centered Care Teamwork and Collaboration Evidence-based Practice Quality Improvement Safety Informatics The paper must address the patient situation in regard to Transition Theory and answer the question(s): ( Look up Meleis’ Theory of Transition on google) a) What transition(s) is the patient in? b) What are the facilitators and inhibitors? c) What is or was the patient’s response? d) What was the nursing intervention? Required Elements for the Capstone Paper The paper will be a minimum of 12 pages in length, excluding title page and references. The paper must be constructed according to APA format 6th Edition. Headings are to be included (see below) and reference citations in the text of the paper. Introduction: Introduce the patient problem clearly and completely. State its significance. Describe the significance of this clinical problem as related to the one identified IOM/QSEN nursing competency: Why is it important to study/explore this issue or concern? 2) Review of the Research Literature: Complete a review of the literature and relate the patient problem to an IOM/QSEN competency. Therefore: Review literature in two areas (separate these by headings): 1. Patient problem 2. Nursing intervention / (What does the literature suggest as best practice to improve safety and ensure quality care?). Cite at least 5 references from the nursing literature (nursing journals) that were written within the last 5 years – at least 3 of these references need to be research-based. Examples found on Blackboard under course resources Textbooks and Wikipedia cannot be used as references. 3) Case Example: The case example must exemplify the clinical problem related to the identified IOM/QSEN competency. Apply Transition Theory to the case example. You may use references from class for your description of the theory. In the case example, explicitly identify the transition, facilitators, inhibitors, and anticipated or actual patient response. 4) Conclusion: Integrate nursing implications from your literature review and relate them to your patient’s problem. Include nursing implications in these three areas: Practice: What is the best nursing practice for this issue? Education: What are the current guidelines for educating patients / families about care for this problem? Research: What are the priorities for further study (from a nursing perspective) related to this problem? TURNITIN: Papers must be submitted in TURNITIN (under assignments on Blackboard) as a draft and final version prior to submission to clinical faculty. Students should attach a copy of their report from TUNRITIN with their emailed copy of the draft and final version to their clinical instructor. A score of 10-15% or less is ideal including the reference page. (See grading rubric). Criteria for Grading Capstone Paper The paper is a requirement for course completion Failure to receive a grade of 80 on the paper will result in unsatisfactory completion of the Capstone Project and failure of Nursing 455. The identification of a patient problem, submission of a draft version and timely submission to clinical faculty of the completed paper are all required. 2.) A complete draft of paper must be submitted to the clinical faculty as a word attachment or in hard copy per faculty choice. Clinical faculty must receive the draft via email with the TURNITIN report also attached. If this is not accomplished points toward the final paper will be deducted as follows: DUE WEEK OF Nov. 11 Failure to submit a completed paper on the due date will result in a 20 point deduction from the final paper grade. A draft requiring considerable corrections and/or deemed unacceptable by the instructor will be returned to the student with 20 points deducted from the final grade.
