Complete an individual care plan for a hypothetical individual diagnosed with the disability/illness you selected for your course project. Use the form provided or develop your own ensuring that you include all the content required for a quality plan of care. Write a description of a patient/client including; name, age, gender, diagnosis, reason for needing recreation therapy services. Describe the individual’s needs and strengths/skills based on facts about the population. Use your book to guide content. Write Therapeutic Recreation goals (long term) and objectives (short term) for a person diagnosed with the disability/illness? These must be written in the format including condition, behavior and criteria. It is important your goals fall within the therapeutic recreation scope of practice. Exhibit 4.1 (pg. 114) in your textbook will give you good goal ideas within each category. Using activity analysis, identify the Recreational Therapy interventions and adaptations needed to help a person reach the goals/objectives. Identify additional service team members and explain how they will contribute to the wellness of the patient/client/family. (Social Work, Nutrition, Physical Therapy, etc.) Record any other information relevant to the plan and sign and date plan with professional title. Post the care plan on the discussion board in PDF format and title the thread with the diagnosis and age of your hypothetical client/patient – “Cerebral Palsy – Young adult”
