Health Protection, Prevention, and Promotion Plan
In this final assignment for the Project, your small group will finalize the Project with a comprehensive plan and strategies for health prevention/promotion with the goal of improving this family’s health outcomes. Also include educational opportunities for the family to make changes needed to realize positive health outcomes, ways in which to maintain adherence to these changes, and any supportive resources that may be needed.
