In this assignment, you will be faced with improving documentation for one of the population health issues that you identified for your Unit 5 assignment, Vila Health: Population Health. You will choose one of the population health issues, then analyze and design appropriate documentation for services that could be or are being provided. Assignment Instructions You will use the materials provided in the Vila Health: Population Health activity to determine where standardized terminology will be needed when providing services related to the specific population health issue. This may involve modifying existing documents or creating new documentation with all appropriate standardized terminology used. For this assignment, you will: Identify at least four areas of potential standardization for the documentation process. Select the correct standardized documentation process for each of the areas identified for improvement. Create a how-to-guide that illustrates the appropriate use of documentation for a health care organization. Use appropriate visual aids for each one of the four areas summarized in the how-to-guide. Writing Requirements Length: At least 4–6 double-spaced pages, excluding the cover page, reference list, and how-to-guide. Include page numbers, headings, and running header. References: At least five peer-reviewed resources, formatted to current APA guidelines. Formatting: Use correct APA style and formatting, paying particular attention to citations and references.
