Name the organization and program that you plan to evaluate. Provide basic background information about the organization including the type of organization, its size, purpose and any other information that you feel is relevant. Provide basic backgroiund information about the organization’s specific program that you plan to evaluate. What are its purpose, goals, the activities that it performs and the expected outcomes for the program? Who are the organizational and program leaders that you will work with to carry out the evaluation. What connections do you have with them? If you have no connections, how do you plan to establish them? What will be the main purpose of your evaluation as you understand it now?
